Home Health NursePeoria, Illinois needed for a contract position in Peoria, IL6/29/2020 for 13 weeks RN would be responsible for completing 5-6 visits daily, or 25 per week8 Hour days M-FNo weekends, no holidays, no call.The RN Case Manager is responsible for decision-making regarding the home health care the patient receives during their episode of care.
